The debate over the future role of the rural hospital is taking place within the larger context of a health care environment in the United States that is demanding the reinvention of the American hospital. These changes have forced hospitals to shift their emphasis from filling acute inpatient care beds to providing a more diversified set of services through linkages with other institutions and provider groups. Rural hospitals often serve as a foundation for health care delivery in rural communities yet struggle to overcome the effects of troubled local economies, shortages of health professionals, and public policy inequities. This project reviews key trends and challenges facing rural hospitals since the Balanced Budget Act of 1997 from the perspective of their structure and organization, scope of services, financial sustainability, quality improvement, and strategic linkages with other entities.
